Facility-dually certified for skilled and intermediate care. (This is unique
because most facilities only have a certain number of Medicare certified beds)

Admission Procedures---Factors considered for admission are
diagnosis and sex. Level of care is determined by a Physician. New admissions must have
a current history and physical and admission orders.
Admission Requirements---Residents are admitted, assigned a bed,
and rendered services without regard to race, creed, national origin, age and
disability.
