Hearthstone Place Mission Statement

The mission of Hearthstone Place is to provide a Tradition of Caring for the healthcare needs of Todd and surrounding counties and to become the Pennyrile areas leading healthcare facility by catering to the needs and desires of the geriatric and special needs population.

 

Introduction

Located in Elkton Kentucky, Hearthstone Place is Todd County's only Personal Care, Nursing Facility and Skilled Nursing Facility. We are equipped and staffed to accommodate 112 residents.

The building is constructed and equipped to meet the health care needs of Todd County and the surrounding communities. We are proud of our reputation and will continue to provide a "Tradition of Caring".

v     Type of facility�52 Personal Care, 60 (NF) Nursing Facility-dually certified for skilled and intermediate care. (This is unique because most facilities only have a certain number of Medicare certified beds)

v     Number of Beds� 52 PC and 60 NF

v     Admission Procedures---Factors considered for admission are diagnosis and sex. Level of care is determined by a Physician. New admissions must have a current history and physical and admission orders.  
 

Admission Requirements
---Residents are admitted, assigned a bed, and rendered services without regard to race, creed, national origin, age and disability.

--Private and semi private rooms, 2 room suite
--All rooms located on one level
--Located within 25 miles of 3 hospitals
--In-patient and out-patient rehab
--Local PACS and ambulance transportation
